Preparing for your Permanent Cosmetics appointment

Thank you so much for choosing VAULT COSMETICS @ Prodigo for all your Cosmetics Enhancements & Fine Line Tattoos. It is an honor for us to be selected and trusted! Please carefully, read, review, print and sign the necessary forms below. Do this prior to your appt. so we can address any additional questions you may have. 

ALL CLIENTS- It is very important to review "Can I get a Cosmetic Enhancement" document. You may need Medical Clearance or Pre-Medication. Most everyone is a candidate. We take every safety precaution so you have an amazing experience! Do not drink coffee or alcohol the day of procedure. Avoid alcohol the night before procedure. Stop using Retinols and exfoliating acids in the area to be tattooed 1 week prior to procedure.

BROW CLIENTS- If this is your first tattoo with us, please send pictures of your brows (never tattooed before or if you have an existing tattoo) to vaultprodigo@gmail.com. Please put in the subject box your name and appointment date/time. 

LIP CLIENTS- If you have an existing tattoo in these areas. please send picture to vaultprodigo@gmail.com. Please put in the subject box your name and appointment date/time. 

Be sure to exfoliate lips, lightly and moisturize for 3 days prior to appt.  Chapped skin cannot be tattooed.

PRE-MEDICATION (Valtrex, Lysine, Valaciclovir, Famciclovir, etc.) is required 1 week before and after procedure if you have ANY history of cold sores / shingles. 

EYELINER CLIENTS-  If you have an existing tattoo in these areas. please send picture to vaultprodigo@gmail.com. Please put in the subject box your name and appointment date/time. Remove all makeup / mascara / extensions from eyes prior to your appt.  If a contact lens wearer, please remove and don't forget your eyeglasses so you can drive home.

Discontinue using any lash growth serums 1 week prior to eyeliner procedure until 2nd session has been completed. Can resume these products 2 weeks after completed procedure.  Continued use of these products thins the skin and will cause more and unnecessary bleeding. The procedure will not be able to be performed. 

(SMP) SCALP MICROPIGMENTATION CLIENTS- Please send pictures of area of concern on the scalp (scars, full head, targeted area) to vaultprodigo@gmail.com. Be sure to exfoliate scalp area to be tattooed, lightly moisturize for 3 days prior to appt.  Chapped ,dry patchy skin cannot be tattooed.

FINE LINE TATTOO CLIENTS-  After scheduling your appointment, please send an email to vaultprodigo@gmail.com indicating what graphic interests you. Fine line tattoos use only black pigment, no color, some shading possible, and very thin, delicate lines. Please read over the "Tattoo" Consent Form below. We will have a copy for you to sign in the shop. Must be 18 or have a consenting adult (parent/guardian) present to sign for approval.

SCAR CAMOUFLAGE CLIENTS-  After scheduling your appointment, please send pictures of the scar(s) to  vaultprodigo@gmail.com. One of our talented Tattoo Artists will reach out to you to ask any additional questions, if needed.
